Thank you for choosing Branchville Animal Hospital to care for your pet. Filling out the appropriate form prior to your appointment will greatly assist us in adding you and your pet into our system.
New Client Form:
Please familiarize yourself with our “New Client Form” (linked below) prior to arriving at Branchville Animal Hospital for you or your pet’s first visit. The up-to-date information you provide us will be resourceful when receiving reminders, alerts, and updates regarding your pet’s health and medical record.
Surgeries:
On the day of your pet’s surgery, one of our technicians will speak to you beforehand about additional services you may choose to add to your pet’s procedure (located on the “Surgery Release Form” and “Dental Release Form” linked below).
These services can provide an additional degree of safety (i.e. bloodwork, IV catheter + IV fluids, anti-nausea medication, etc.) for your pet while under anesthesia.
We can also provide services such as toe nail trims, anal gland expressions, microchip insertion, and ear cleaning while under anesthesia. While these services are often routine, performing them under anesthesia can reduce stress for your pet during their visit.
Unless your pet has a specific health concern that mandates additional precautions, the optional services are selected at the owner’s discretion. Because these services are client-driven, they have been added to our surgical release form over the years as an aid in pre-surgery preparation, recovery, and post-operative care.
Owners are free to either select or decline services based on their pet & their pet’s lifestyle.
